Orthodontic expanders are appliances used to widen the upper arch or jaw for selected spacing, crowding, and crossbite concerns. How Palatal Expanders Work

A palatal expander applies gentle, controlled pressure to the upper jaw to gradually widen it over time. In growing children and adolescents, the midpalatal suture has not yet fully fused, allowing the jaw to expand relatively easily. The expander is typically activated by the patient or parent using a small key, with each turn providing a tiny amount of expansion.

Non-Surgical Adult Palatal Expansion

In adults, the midpalatal suture has fused, making traditional expansion more challenging. Dr. Eljach evaluates miniscrew-assisted expansion using temporary anchorage devices (TADs) for selected adults. These small biocompatible mini-screws can provide additional anchorage during planned expansion.

This approach is often called miniscrew-assisted rapid palatal expansion, or MARPE. Adult candidacy depends on anatomy, periodontal health, treatment goals, and diagnostic records. Dr. Eljach explains the expected role of expansion, possible limitations, and alternatives, including coordinated surgical care when appropriate.

When Is Expansion Needed?

• Crossbite correction — When upper teeth bite inside lower teeth
• Crowding — Creating space for permanent teeth to erupt properly
• Narrow arch — Widening a constricted upper jaw
• Pre-orthodontic preparation— Creating space before braces or aligners

Expansion for Children

An early orthodontic evaluation can identify developing crossbites, crowding, eruption concerns, and narrow arch relationships while growth remains. Evaluation does not mean every child needs an expander or immediate treatment. Dr. Eljach will review development and explain whether monitoring or treatment is appropriate.
